How AI is Revolutionizing My Practice as a Podiatrist

When I first began practicing podiatry over 20 years ago, I never imagined that artificial intelligence would become valuable tool in my day-to-day clinical work. At the time, my focus was on treating heel pain, bunions, diabetic foot ulcers, and the occasional sports injury. My tools were a combination of clinical training, diagnostic imaging, patient history, and intuition. But in recent years, the integration of AI into podiatry has transformed how I diagnose, treat, and manage patient care.

I want to share how this technology is enhancing our ability to deliver more accurate diagnoses, offer personalized treatments, and improve overall outcomes – while saving time and reducing human error.


AI  Diagnostic Imaging


One of the most immediate ways AI is helping podiatrists is through imaging. Whether it's an X-ray, MRI, or CT scan, reading and interpreting these images is a critical part of diagnosing foot and ankle problems. With the help of foot and ankle problems. With the help of AI-driven image analysis, I can now detect subtle fractures, joint abnormalities, or soft tissue injuries that might have been easy to overlook with the naked eye. 

AI algorithms trained on thousands of annotated images can spot patterns consistent with plantar fasciitis, osteoarthritis, or even early signs of Charcot foot in diabetic patients. These tools act as a second set of eyes, confirming my suspicions or occasionally flagging something I might have missed. This has made the diagnostic process faster and more reliable.


Predictive Modeling for Diabetic Foot Care

As podiatrists,  we pay special attention to diabetic patients because they are at high risk of developing ulcers and infections that can lead to amputations. One of the biggest challenges has always been predicting which patients are at the highest risk so we can intervene early.

Now, AI systems are helping us do just that. Using data from patient records – such as wound history, blood sugar levels, vascular status, and gait analysis – AI models can calculate the probability that a patient will develop a foot ulcer. This predictive capability enables us to tailor preventive strategies more effectively, such as recommending custom orthotics, adjusting footwear, or scheduling more frequent follow-ups.

We're no longer simply reacting to problems; we're anticipating them.


Gait Analysis and Biomechanics

Biomechanical assessment has always been a core part of podiatry. AI-powered gait analysis tools are now giving us new insights. These systems use cameras and sensors to track a patient's walking pattern and compare it against vast datasets of normal and pathological gaits. 

In the past, gait analysis was largely subjective – based on our visual observation and perhaps some video playback. Today, AI can quantify asymmetries, measure joint angles in real-time, and even suggest orthotic modifications. For patients recovering from injury or surgery, we can use this technology to monitor their progress objectively and fine-tune rehabilitation plans.


Custom Orthotics

Designing custom orthotics is part science, part art. AI is helping refine both. With digital foot scanning and AI algorithms, I can create a detailed 3D model of a patient's foot, taking into account pressure points, arch height, and gait characteristics. The AI can then suggest orthotic designs that are biomechanically optimized for the patient's condition.

Not only does this result in better-fitting and more effective orthotics, but it also reduces turnaround time. Instead of waiting weeks for a lab to mold and return a product, we can now use in-office 3D printing integrated with AI design tools to produce orthotics in days- sometimes even hours.


Administrative Efficiency and Documentation

Though less glamorous, another huge benefit of AI is in administrative work. Clinical documentation, coding, and insurance claims take up a disproportionate amount of time. AI-powered voice transcription and smart EHR (Electronic Health Record) systems can auto-generate notes, suggest ICD-10 codes, and even flag incomplete records.

This allows me to spend less time on paperwork and more time focusing on patients. It also reduces the  chance of clerical errors that can lead to claim rejections or compliance issues.


Patient Education and Engagement

AI is also enhancing the way I communicate with patients. Chatbots and mobile apps powered by AI help educate patients about their conditions, remind them to perform stretches or exercises, and even monitor compliance. For example, an app might track a diabetic patient's daily foot checks and alert them – or me – if any issues arise.

This type of digital engagement keeps patients more involved involved in their care, leading to better outcomes and fewer complications.


Challenges and Looking Ahead

Of course, AI isn't a magic bullet. It requires quality data, careful implementation, and a strong understanding of its limitations. It will never replace the clinical judgement that comes form years of experience. But as a podiatrist, I see AI not as a replacement – but as an enhancement.

It's helping us become more precise, more proactive, and more patient-centered. In a field like podiatry,  where early detection and personalized care make all the difference, that's a game-changer.

While the condition is relatively uncommon, it can affect individuals of various ages, from children to adults, and is important to address to prevent long-term complications.  


What Is Popping Peroneal Syndrome?

The peroneal tendons play a critical role in ankle stability and foot movement. These tendons run behind the fibula, a bone on the outer side of the lower leg, and attach to specific points on the foot. Their primary function is to help with foot eversion (turning the sole outward) and plantarflexion (pointing the toes downward).  


In PPS, one or both peroneal tendons dislocate from their normal position behind the fibula. This dislocation causes the tendons to "pop" forward over the bone, resulting in the characteristic symptoms of the syndrome.  


Symptoms of Popping Peroneal Syndrome  


Common symptoms include:  

- A popping sensation or sound on the outer side of the ankle, particularly during movement.  

- Tenderness or discomfort in the affected area.  

- Ankle instability or a feeling that the ankle may give out.  

- Swelling or inflammation around the tendons.  



What Causes Popping Peroneal Syndrome?


Several factors can contribute to the development of PPS, including:  


1. Shallow Fibular Groove:  

   The peroneal tendons are naturally housed in a groove behind the fibula. If this groove is too shallow, the tendons are more likely to dislocate. This anatomical variation may be present from birth.  


2. Weak or Torn Retinaculum:

   The peroneal retinaculum is a band of tissue that holds the tendons in place. If this structure is weak or damaged—due to injury or congenital conditions—it can fail to restrain the tendons, leading to dislocation.  


3. Tendon Tears or Damage:  

   Although less common, a tear in the peroneal tendons can cause them to shift out of place, resulting in the popping sensation.  



Diagnosing Popping Peroneal Syndrome  

A thorough physical examination is key to diagnosing PPS. A physician may perform specific stress tests to reproduce the popping sensation and pinpoint the dislocation. Imaging studies, such as MRI or ultrasound, can provide detailed insights into the tendons' condition and help identify any tears or anatomical abnormalities, such as a shallow groove.

Hey, this is Dr. Lam here, just came out of surgery, taken out the medial sesamoid -- medial sesamoiditis on a patient that's a diabetic and she keeps on having this pre-ulcerative lesion down there at the medial sesamoid.

So having a stand up CT scanner has been a great addition to our practice. We're able to see pathology at weight bearing, we are able to cut up the foot in different sections for us to see the pathology for the detail.

This was impossible prior to having this CT scanner. So great diagnostic tools with the suspicion for a problem we could figure out a lot of different pathologies that others may have missed.


So at the Family Foot & Leg Center "we keep you walking" -- only the best for our patients.


Well this is a case of an enlarged medial sesamoid on a patient. Medial sesamoid being right here. It's hard to see on the plain film, you can look at the lateral view right here you can't really see much of that either.


Someone else did the bunionectomy many years ago and there you go,


I'm not here to make it beautiful but she has a lot of pain sub medial sesamoid. We're gonna go all the way over here, we're gonna see on the CT scan why she's feeling the pain right here.


See that sesamoid, it's really enlarged and we did ultrasound scans about three millimeters from the surface of the skin so having the ability to be able to go through a CT scanner like this via weight bearing it is very important for patient care and we have this available at Family Foot & Leg Center for our patients, we're able to take care of this patient immediately in the office via excision of that medial sesamoid.


This medial sesamoid measured about 2 cm,as you may or may not know these bones are not supposed to be that big, they're supposed to be probably about one centimeter at most. This is the bone we're talking about, medial sesamoid right here, there are other drawings in here but this is the medial sesamoid.


As you can see, it's supposed to be nice, round, hers is very degenerated and again this is how it's supposed to look. That is very normal, we all have this -- there's enough fat, supposed to be enough fat right here to cushion this but when it's enlarged and arthritic -- and arthritic because it's probably a repeat trauma or just arthritic from years of arthritis in the first metatarsal phalangeal joint, the big toe joint, if this gets enlarged when this gets enlarged right here, the sesamoid becomes almost like a rock pushing -- trying to push through the bottom of the skin and that's happening in our diabetic patients.


So again having a CT scan and having the ability to see the CT cutting the foot into many pieces while you're standing, it's great for us diagnostically in order to help treat patients effectively.
